Please provide the following information required for setting up your Blackboard course(s). You can request up to four new courses with this form. If you need to set up more than that, please submit additional forms.
Upon submitting the form, you will receive an e-mail message confirming the request. In this message, you will find the name of the Blackboard support personnel that has been assigned to your request. If you have questions about the request, please contact the person listed on the e-mail. If you do not receive the confirmation e-mail, please e-mail teachtech@ilstu.edu. Please do not re-submit the form.
Note: If you are teaching two or more sections of the same course and would like to have a separate Blackboard course for each section, please use a single row for each course you’d like. If you prefer to teach multiple sections from a single Blackboard course, please use a single row and put all the section numbers.
